How to Authorize Pinnacle as Your Survey Vendor

Find full instructions on the Home Health CAHPS® website. Please note that if this is not completed, we will be unable to submit your quarterly CAHPS® data to CMS, jeopardizing compliance.

(OPTION 1) I Am New to CAHPS® Instructions

  1. Go to the Home Health Care CAHPS® survey website.

  2. Click on the “For HHAs” tab.

  3. Select “Register for Login Credentials.” (The contact person is the Survey Administrator.)

  4. After registration is complete, log in using the top right-hand corner link.

  5. On the black banner, go to the “HHA Dashboard” tab.

  6. Scroll down to the Vendor Authorization section.

  7. Select Pinnacle Quality Insight from the drop-down list.

  8. Select the correct start date. (This will be the beginning of the next quarter.)

  9. Leave the end date blank.

  10. Select the home health agency CCN associated to the authorization.

  11. Click the “Submit” button.

(OPTION 2) I Am Not New to CAHPS® & Need to Switch Vendors

  1. Go to the Home Health Care CAHPS® Survey website.

  2. Click “Login” at the top of the page. The person who has previously signed up for this website should sign is as the survey administrator. (If you need further login help, contact the Home Health Care CAHPS® Survey Team at hhcahps@rti.org or call 866.354.0985.)

  3. On the black banner, go to the “HHA Dashboard” tab.

  4. Find the Vendor Authorization section.

  5. Select “Change the start/end date for an existing vendor authorization” from the Select Actions drop-down list.

  6. Select the correct end date for your current authorized vendor. (This will be the end of the quarter with the previous survey vendor.)

  7. Click on the “Submit” button.

  8. Select “Change/switch to a different vendor” from the Select Actions drop-down list.

  9. Select Pinnacle Quality Insight from the drop-down list.

  10. Select the correct start date and leave the end date blank. (This will be the beginning month of the quarter with Pinnacle.)

  11. Select which home health agency/agencies apply to this application.

  12. Click the “Submit” button.
